Southwest CHC offers recipients the choice of the Moderna, Pfizer or Johnson & Johnson vaccine. We are currently offering the COVID-19 vaccine for individuals ages 5 years and older.
You can expedite your experience by scheduling an appointment but please know that this is NOT a requirement. If you do not have a VAMS account, our staff can create one for you. Federal Identification is preferred but not required.
Scheduling a Vaccine Appointment
You can schedule your COVID-19 vaccine through the Centers for Disease Control and Prevention’s (CDC’s) Vaccine Administration Management System (VAMS) or by calling 877-918-2224. You may also walk-in without an appointment. Please do not call Southwest CHC to book an appointment.
If you schedule through VAMS, you should return to VAMS to schedule your second dose.
Please fill all required fields attesting that you are currently eligible and provide the requested information. This will enable State of CT staff to upload your information into VAMS. Within 24 hours each individual will then receive an email from the CDC/VAMS telling them to now register in VAMS and select a vaccination appointment. Make sure to check your email Spam/Junk folders for the email.
