NO APPOINTMENT, NO PROBLEM! WE ARE NOW OFFERING WALK-UP TESTING AND VACCINATIONS AT OUR SAME-DAY CARE LOCATION AT 1020 FAIRFIELD AVE, BRIDGEPORT, CT
Southwest CHC offers recipients the choice of the Moderna, Pfizer or Johnson & Johnson vaccine. We are currently offering the COVID-19 vaccine for individuals ages 5 years and older.
You can expedite your experience by scheduling an appointment but please know that this is NOT a requirement. If you do not have a VAMS account, our staff can create one for you. Federal Identification is preferred but not required.
COVID TESTING IS NO LONGER FREE OF CHARGE
Patients will be required to show proof of insurance and will require sliding fee FPL registration. If a patient is uninsured, they will be charged according to appropriate sliding fee scale rates.

You do not need to be a patient to receive services relating to COVID vaccination and testing.
After you are tested, you will be notified of your results.
What is Coronavirus Disease 2019 (COVID-19)?
Coronavirus disease 2019 (COVID-19) is a respiratory illness that can spread from person to person. The virus that causes COVID-19 is a novel coronavirus that was first identified during an investigation into an outbreak in Wuhan, China.
What are the symptoms of COVID-19?
Patients with COVID-19 have had mild to severe respiratory illness with symptoms of
- Fever
- Cough
- Shortness of breath
In severe cases, patients have developed pneumonia in both lungs, multi-organ failure and in some cases death.
How can I help protect myself and family?
People can help protect themselves and/or family from respiratory illness with everyday preventative actions.
- Avoid close contact with people who are sick.
- Avoid touching your eyes, nose and mouth with unwashed hands.
- Wash your hands often with soap and water for at least 20 seconds. (Use an alcohol-based hand sanitizer that contains at least 60% alcohol if soap and water are not available.)
