Southwest Community Health Center is a non-profit organization dedicated to providing high quality care that includes medical, dental, and behavioral health services to individuals and families in the Greater Bridgeport area. In addition, Southwest CHC also provides health education, disease prevention programs, community outreach, and enrollment services. We have a variety of finance options available to assure that we accommodate individuals who may not be insured. We strongly believe our clients have the right to a quality of care that meets their own needs regardless of their ability to pay.  Our health care facility maintains a friendly, professional and efficient health care delivery. This locations offers a full panel of services that include internal medicine, women’s health, dental, behavioral health, pediatrics, and HIV/AIDS testing and treatment.

This site concentrates on behavioral health treatment including psychiatric evaluation, treatment, and therapy for children and adults. Clinicians provide individual, family, and group counseling. Patients experiencing substance abuse can participate in the Intensive Substance Abuse and Mental Health program and seek continuing care as well.

As our newest location, this facility offers a family practice model to serve the needs of our community.  This location boasts a diverse, culturally sensative team of medical and behavioral health providers.

1020 Fairfield Avenue (WIC Services)

This site caters to women, infant and children and is known as our WIC program. It provides nutrition education, health care referrals, and breastfeeding promotion and support. A SNAP Outreach worker is accessible and assists clients with SNAP (Supplemental Nutrition Assistance Program)/Food Stamps enrollment.

This location offers immediate and easy access to high-quality health care. If you need help for a non-life-threatening illness or injury, such as a sore throat, a sprain, stitches etc. - please feel free to walk in!

762 Lindley Street - Southwest Family Health Center

The Southwest Family Health Center is located at 762 Lindley Street. The Southwest Family Health Center is a partnership between Southwest Community Health Center and St. Vincent's Hospital which included the acquisition of the Seton's Women's Center. Southwest CHC is proud to bring enhanced services to patients through the integration of a mental health therapist on site along with the continuation of the St. Vincent's Residency Program.

This Family Health Center offers Internal Medicine (adults), Pediatrics (children) and Mental Health (children and adults) in addition to the Women's Center on the lower level.
