APPOINTMENTS

In order to better serve our patients Southwest Community Health Center, Inc. is open 6 days a week and evening hours. We encourage you to schedule an appointment for your visit. Having a scheduled appointment will reduce your wait time. We welcome walk-in visits for patients who are sick and need to be seen immediately.

In order to facilitate your registration, we ask that you provide your insurance information at the time you schedule the appointment. Should you find it necessary to cancel your appointment please give us a 24 hours notice. This will allow us to give the appointment slot to another patient.

On the day of your appointment we will require that you show us identification, proof of address, and insurance information. If you are uninsured you can apply for our Discounted Fee Schedule Program by completing an application and providing proof of available income.
